As an example image I'll use this picture of Josh Kalis :

Step 1 :

First, open the image in Photoshop :

Step 3 :

With the new layer selected, press CTRL+A or use the Rectangular Marquee Tool to select the entire canvas :

Step 4 :

With the canvas selected go to Edit -> Stroke :

Step 5 :

Choose the size of your border. For this picture I'll use a border size of 2 pixels. Then, choose the color of your border and press OK (Leave the other settings as they are)

Painting completely straight lines with the brush tool :

Part 1, creating horizontal and vertical lines :

First, make sure you have the brush tool selected. Then choose a brush tip shape of your choice. To make horizontal lines, hold Shift, click the mouse to start painting and then drag the mouse to the right or left. To make vertical lines, hold Shift, click then drag the mouse up or down. Example :

Part 2, creating straight lines from point to point :

Step 1 :

Open a new document. Use a size of 500x500 pixels to test. First, hover your mouse over where you want to start the line :

Step 2 :

With the mouse hovering where you want the line to start, click the mouse so a dot appears and then press and hold the shift key. With the shift key held down, move your mouse to where you want the line to end. Click with the mouse and the line will appear :

Heres a tutorial on creating your own brush patterns :

Step 1 :

Open a new document. For this tutorial we'll be using a document size of 100x100 pixels and a transparent background :

http://img248.imageshack.us/img248/9706/step1bs2.png

Step 2 :

Zoom in on the canvas at 500% :

http://img176.imageshack.us/img176/3558/step2gk0.jpg

Step 3 :

Find a high res image on Google, or create a simple pattern you want to use as a brush. This is when your artistic skills take over. I'll use a simple pattern i made myself for the tutorial :

http://img85.imageshack.us/img85/9134/step3nl3.jpg

Step 4 :

Press CTRL+A or use the Rectangular Marquee tool to select the entire canvas :

http://img176.imageshack.us/img176/4879/step4hm9.jpg

Step 5 :

With the entire canvas still selected, go to Edit -> Define Brush Preset :

http://img176.imageshack.us/img176/3970/step5sm4.jpg

Step 6 :

Name your brush. I'll name mine Design 1. Then press OK :

http://img248.imageshack.us/img248/8701/step6cx2.jpg

Step 7 :

To use your brush make sure your using the brush tool. Then go to the Brushes window/tab (If it is not on your screen, go to Window -> Brushes) and then go to Brush Tip Shape. Scroll down if needed and find your brush. :

Have fun creating your own brushes, but remember that not all of them are going to turn out great, so its best you do some trial and error until you find what type of designs work best :) Its also useful if you want to be able to quickly place a decal on a poster etc. Don't be scared too make brushes larger than 100x100 pixels, Photoshop's maximum brush size is 2500x2500 pixels.

I think the shortcuts are CTRL+A for select all, and then CTRL+SHIFT+S for stroke. I usually like 3 pixels black, 2 pixel white, 1 pixel black borders, makes more detailed banners look cleaner, but good tutorial for the noobs.
